“It’s terrible, son…I wish I could take my head off my body.”

My father Mario could not be more explicit when he suffered a migraine. His recurrent attacks, worse during his thirties and forties, took a heavy toll on him, as a breadwinner in the prime of his career, and his family as well.

A web-based study of 1,000 women and men with Chronic Migraine showed that respondents cancelled plans more than four times per month on average and reported reduced involvement in family activities nearly seven times per month on average. They also missed a date or social event with their partner nearly three times per month on average and 20% missed a family vacation.

There are new developments for the prevention and treatment of Headaches.

Levels of Anti-calcitonin gene related peptide (CGRP) rise in samples of jugular venous blood during a migraine attack, which can be reduced by Sumatriptan in experimental animal models. At present specific antibodies against CGRP are being studied in clinical trials for the acute and chronic treatment of cluster headaches—a potentially innovative preventive therapy.

Neuromodulation regulates the nervous activities through the control of the physiological levels of neurotransmitters. Transcutaneous supraorbital nerve stimulation (Cefaly) was approved by the Food and Drug Administration (FDA) for the prevention of migraine with and without aura. A randomized, double blind, multicenter study of 157 patients was done to assess implanted Occipital Nerve Stimulation (ONS) that showed that headache days were reduced by 6.7 +8.4 days overall and 7.7+8.4 days in chronic migraines. The External vagal nerve stimulation (VNS) was studied in two European studies in 2014 and at the present time, studies in the USA are being implenmented.

Sumatriptan (Imitrex), an effective drug with oral, injectable and nasal spray presentations, could soon be also available in a breath-actuated delivery of a powder using the OptiNose device and an iontophoretic transdermal form.

Feliz Domingo!
